Thank you for participating in the Indiana Doula Conference. Your safety and well-being are important to us. Please carefully read the following General Liability Release and Waiver before signing.

To capture and share the experience of this event, we will be taking photographs and video recordings of attendees, presenters, and activities. Please read the following release and sign below to indicate your consent.

INDIANA DOULA CONFERENCE GENERAL LIABILITY RELEASE & WAIVER

I acknowledge that my participation in the Indiana Doula Conference (“the Event”) is voluntary and that I assume all risks associated with attending, including but not limited to physical injury, illness, property loss, or any other unforeseen incidents that may occur during my participation.

I hereby agree to the following terms:

  1. Assumption of Risk – I understand that participation in the Event may involve certain risks, including but not limited to risks associated with traveling to and from the venue, interacting with other attendees, and engaging in conference activities. I voluntarily assume full responsibility for any risks of loss, damage, or personal injury, including illness, that may be sustained as a result of attending the Event.

  2. Release of Liability – I release and hold harmless the Indiana Doula Conference, its organizers, sponsors, vendors, venue, affiliates, employees, volunteers, and agents from any and all liability, claims, demands, or causes of action arising from my participation in the Event, whether caused by negligence or otherwise.

  3. Medical Treatment – In the event of a medical emergency, I authorize the Indiana Doula Conference to seek medical assistance on my behalf if I am unable to do so. I acknowledge that I am solely responsible for any medical expenses incurred as a result of participation in the Event.

  4. Code of Conduct Compliance – I agree to conduct myself in a respectful and professional manner throughout the Event. The Indiana Doula Conference reserves the right to remove any attendee who engages in disruptive, inappropriate, or unlawful behavior without refund or recourse.

  5. Indemnification – I agree to indemnify and hold harmless the Indiana Doula Conference and its affiliates from any claims, damages, or expenses (including reasonable attorney’s fees) arising from my actions or participation in the Event.

Acknowledgment of Understanding – I have read this General Liability Release and fully understand its terms. I acknowledge that by signing this waiver, I am waiving certain legal rights, including the right to sue the Indiana Doula Conference and its affiliates.

INDIANA DOULA CONFERENCE PHOTO & VIDEO RELEASE FORM

Thank you for participating in the Indiana Doula Conference. To capture and share the experience of this event, we will be taking photographs and video recordings of attendees, presenters, and activities. Please read the following release and sign below to indicate your consent.

I, the undersigned, grant the Indiana Doula Conference and its authorized representatives the irrevocable right and permission to use photographs and/or video recordings of me taken at the Indiana Doula Conference. This includes, but is not limited to, the use of my image, likeness, voice, and any quotes attributed to me for promotional, educational, and commercial purposes.

I understand and agree to the following:

  • My image, video, or audio recordings may be used in promotional materials, social media, website content, advertising, and other forms of communication related to the Indiana Doula Conference and affiliated organizations.

  • The Indiana Doula Conference retains full rights to these materials and may edit, distribute, or publish them without additional consent or compensation.

  • I waive any rights to inspect or approve the final product in which my likeness appears.

  • I release the Indiana Doula Conference, its organizers, affiliates, and any third parties acting on its behalf from any claims, damages, or liability related to the use of these materials.

I acknowledge that my participation in the conference is voluntary and that this agreement is binding upon me, my heirs, legal representatives, and assigns.

If you do not wish to be photographed or recorded, please check the “I DO NOT consent…” box on Eventbrite. Every effort will be made to accommodate your request.
